This is one of my favourite movies. I'm pretty sure the ending means it was all just a dream he was having while he was dying in the jungle, but some people say the ending is also part of the dream, so I don't know. The part I don't get is how it cost them $25 million to make though, there's not many actors and basically no special effects other than a few simple practical effects.

Jacob gets mortally wounded in Vietnam. As he dies he enters a sort of purgatory (although more accurately the director has said he based it on the Tibetan Buddhist concept of bardo). All the demons are manifestations of him still clinging to attachments of his earthly life, like his wife and kid. The scene with the chiropractor explains quite explicitly. Eventually Jacob manages to let go of these attachments and thus manages to move on from this in between death state and goes to heaven proper.
