the helicopter shadow was matted out originally. just ended up in the TV version and then the home releases. sort of like the 16:9 Buffy the Vampire Slayer where you can see filming equipment off to the sides.

I noticed another mistake. When the camera shows the story Jack is writing, you see that the prop person just typed the same sentence over and over again. I wonder how Kubrick didn't notice that.
