OK, so The Rock lost his muscles because he stopped roiding for medical reasons, fine.

But this is an over-the-top, deliberately unrealistic, videogame-y setting. He literally plays an exaggerated videogame character/avatar, where stylization is expected.

They couldn't have given him a body suit? Just like the one that he wears for live action Moana, where his body is meant to be inhumanly large, larger than Dwayne could achieve with steroids even back in his prime. Moana is an adaptation of a cartoon, and the character is meant to be superhumanly large and buff, without trying to emulate some "achievable" physique. So they gave him a body suit for Moana and there's probably some CGI enhancement too (remains to be seen).

But here, in the same situation where they COULD get away with an "unrealistic" body, they just chose to film the deflated Dwayne in his current natural state? With pathetic long sleeves to try to hide that he isn't huge anymore? Why?
