Without the blood protection Voldemort would have killed him during the Summer. And he couldn't have forced them to treat him well because in order or the blood protection to work they had to have accepted him into their house of their own free will.

Dumbledore could do occlumency aka reading minds. He bothered to do it with Voldemorts uncle when he was in Azkaban but never bothered to do it to a person he'd knew well and was in his order, if not just to confirm the things he thought about Sirius.
