Gregg Araki is good and relatively accessible, at least I can pretty easily recommend The Doom Generation, Nowhere and Mysterious Skin (his most popular film). Pre-Doom stuff is very formalistic while Splendor and especially Ka-Boom can be way too over the top with camp (Ka-Boom has literal PowerPoint transitions which I thought were hilarious but I can see someone being baffled).
Everyone's having fun with John Waters although specifically his "transgressive" films kinda suck, don't get the appeal, not even that shocking. Mondo Trasho and Multiplie Maniacs benefit from the black & white image that gives those two a kind of a creepy atmosphere but the colour ones are just awkward nonsense. His actually great run is from Polyester to Cecil B. Demented (we DO NOT talk about A Dirty Shame). Serial Mom is probably his overall best. He tried to make a movie recently but couldn't secure funding, which is weird, we live in an age of A24, NEON, surely they would've been interested. Although it's strange how he gave up after failing to get funding, you made cult movies on a budget of three rotten eggs nigga.
With melodramas / dramas, Call Me by Your Name is popular, but I thought Guadagnino's Queer adaptation was the better film and that got us the funny "Daniel Craig Queer" picture. All of Us Strangers is a good recent one. I though A History of Sound from this year was good, even tho the reviews are mixed. Pedro Almodovar is a very obvious choice although most of his filmography doesn't actually deal with the gay subject matter, at least as the "main plot", he loves his woman stories. Law of Desire is one of few that does and it's pretty good.
For less accessible you can try some Rainer Werner Fassbinder ones, but his movies are harsh and demanding. There are people like Alain Guiraudie and João Pedro Rodrigues from more or less contemporary arthouse I guess.

You got John Waters backwards, the trashy stuff is his peak since his dialogue works best recited by stoned hippies. By the time he was doing Serial Mom his whole thing had become a Gen X cliché (I like that movie but I think it’s his weakest.) Also Gregg Araki is pretty stupid and his best movies are the ones where he’s relying on external source material like Mysterious Skin. Guiraudie is amazing though.
To answer OP’s question I’d throw in the works of Ozon and certain Fassbinder films. The latter is somewhat of an annoying meme among gay guys but he’s still good.
