At some point, for some reason, loki took that baby from his own universe and gave it to mcu's howard stark, because it would get revealed that the starks couldn't have children, and this baby ends up growing as tony stark, what this implies is that rdj has always been victor von doom, except in this timeline, where loki took him as a baby and received a different name. Then, after dying in endgame, tony would be revealed to be the anchor being of the mcu, the sacred timeline, so dr doom from another universe starts acting because of this, since tony stark should have never existed, it's a sin to the existence.
It's a plot very similar to the second season of the show Fringe

I really think Downey has the opportunity to pull a Heath Ledger Joker with this role. I’m a believer in method acting, and Ledger did so much for that part—literally closing himself in a hotel to study and do his own thing. He even changed his fucking voice, which blew me away immediately. Downey is practically doing the same thing. Now, I don't know if he’s pushing himself quite as hard as Ledger did, but who knows? We’ve heard he was heavily involved in writing Doom’s character, and since he’s never played a massive comic book villain role like this before, this could be something truly unique.

An incursion can only be stopped if you destroy one of the Earths before both hit. But that is only a temporary reprieve as they keep happening. But it turned out Rambal was an alias of Doctor Doom, who with Molecule Man, created the persona of Rambal to kill all variants of Molecule Man, who had been turned into a universe destroying nuke weapon by the Beyonders and the Incursions were the Beyonders plan B when plan A (blow up the multiverse via Molecule Man) started going south.
