It took centuries for the emperors to even be honest about the fact that they're absolute monarchs, for a long time they maintained the illusion of republicanism and all the procedure and pageantry of the senate. This is why bureaucratic states always, always, always end up kicking the shit out of everyone else they fight. (Well, except for when they don't, but there's argument there.) They have such deep wells of resources and manpower to draw from, their societies have such strong institutions that whoever conquers them just ends up in charge of the same system.

This is why I would say that the core issues were rooted long ago, decades before Caeser. The big problems were:
1. Widespread Political Violence and reprisals
2. The use of armies for internal, political manners and civil war
3. Removing leaders by assassination
It's all these things becoming more and more common, becoming expected. Marching an army on Rome was like pulling the genie out of the bottle, they could never put it back in.

I would also say that a massive problem is the Roman Empire never really figuring out how to truly administrate different regions. These districts were originally ad-hoc classifications, set up by military commanders for logistics, maybe they figured they would rework it after it was conquered, but it just stuck forever.

So much of the Roman Empire was centralized from Rome, but in some areas it was not enough, or too much.

Did you know that the Republican era, HALF of Roman armies were not Roman citizens? They were made from Roman allies and subject states on the Italian peninsula. These dudes paid pretty much no taxes, but were expected to provide troops. Over time, they became full Roman districts and citizens, and this cheapo form of troops vanished.

There's so much to learn about Rome, so many problems, it's so interesting.

Even when they were a republic, they still appointed dictators, willingly or not. During the empire period, you basically needed a full-time dictator just to prevrnt the entire thing from crumbling, and that's during peaceful years.
Another important thing is Augustus' long reign. By the time he died, there were several generations of Romans who got used to have a single central demigod ruler. There was no one around anymore to bring back the republic. They were long dead already from purges.
