The entire scene has the audience on edge that any one of the restaurant patrons could be a killer out for Tony. But it makes no sense that the screen goes dark from the audiences point of view if Tony was killed.Tony is being looked at from Meadows point of view as she walked in, and it was Meadow that was snuffed out.

It was a dream sequence. The POV shots give it away since they're in every other dream sequence. Plus when Tony teleports to his seat and loses his jacket with the jukebox music still playing.

Notice how "moved" Tony was when Carmine High IQ Lupertazzi Jr. told him how he quite trying to be boss because it was more important to be happy.
