One I saw recently was Miller's Cave, Iowa. You rappell down, go through a 6ft hole, then descend 100ft to the bottom. A group of two instructors and five students trained and practiced to go. Turns out the weather was going to snow and rain later in the day oops. When they got there, it was fine and clear, the descent went fine. But then the storm hits and starts pouring a river of freezing cold water through their only exit hole 100ft up. They decided they had to escape asap, but could only go one at a time, and the people that reached the top were supposed to lower additional ropes. The cold water made them numb and develop hypothermia, barely able to move fingers or take off equipment. Two of them die trying to climb out. One from the cold who started screaming frantically as he knew he was a goner and couldn't escape. Another drowned on the ascent as the water pouring down on her was that bad. Several of the students were still trapped in the cave and survived when a rescue team showed up and was able to get them out
