This could possibly be the hellish realm known as Naraka. They have two ways to be reborn again in one of the upper realms. Either pay off their karmic debt through continued suffering or perform pure unselfish acts of compassion before dying that will propel them to one of the upper realms upon rebirth, such as modern day Earth. This last one is why Tabitha was reborn in modern day Earth, because she died after performing a pure unselfish act of compassion (venturing alone into a dangerous area to free the enslaved children in an attempt to save everyone and her family). The boy in white then says "I'm sorry this is the only way" and then pushes her out of the lighthouse, implying the only way for her to be reborn in modern day Earth is to die.Finally only by achieving enlightenment (nirvana) and letting go of all craving and ignorance will they be able to break the cycle of samsara (rebirth) and attain absolute freedom.
