The reason Rue, the storyteller, cares about Nate and Cal, is because they're such an important part of Jules' life. And the reason they're both such an important part of Jules' life is intrisically wrapped up in Jules' nature as a trans woman. The story being told, by Rue, is literally Rue obsessing over the fact that Jules is a trans woman.

Secondly in Rue's story Jules' cares deeply about Rue and so Jules is going to try to save. At which point potential plots branch out in two directions. Jules is successful in bringing Rue back to health, or she isn't. The latter is probably the better story, but either would be ten million times better than what you wrote.
