You might think there's a point of criticism for the film here because this is clearly not what love is, however the way he phrased it then does make her behaviour make sense. If he simply said "I wish nikki would fall in love with me" I imagine the outcome would be much less intense.I interpreted the phone call as the support worker channeling the real nikki inside her own body. Like a psychic mechanism. Wishes are real in this world, that's not a leap.The hospital conversation I didn't understand either. There is no way they would not treat her after a nurse examined her injuries, that's not how emergency departments work.
