He's running from Homelander, he's easily keeping pace and dodging all of the obstacles in front of him, but then he turns and looks back to see if Homelander is behind him. When he looks forward again, he realizes that he's about to ram into a girl and kill her. He tries to slow down but he can't slow down fast enough. So he instead shifts direction, but the harsh change in direction at superhuman speeds leads to him tripping and falling. After which, A Train crashes into a wide variety of obstacles because his body is now moving solely on momentum alone. The whole situation is a lot like speeding with a car. You may SEE you're about to hit something when you're going too fast, but your car can't physically react fast enough to avoid the crash. The series drives this even further home with Marathon Man, who in season 1 and 2 was portrayed as A Train's near equal. The two even raced and A Train ended up barely beating him. In Season 5, we finally see Marathon Man in action, and Soldier Boy is able to bait him into ramming into and killing all of his friends. Marathon Man doesn't even try to stop his deceleration or change his direction because he's an inherently selfish person who isn't going to harm himself for other people.
