It's not called character, it's called avatar. For a reason.
It's not medic's face, it's medic being asked how he wants to change his face. At this point he already had the Bibo face surgery.

You'll get so much out of this game if you just think a little while playing. This isn't your everyday slop and you don't have be high IQ to get it but you do need pay attention. Do yourself a favor.

I don't understand your obsession with jargon I don't understand but what I think (because it is) it's the face Medic had before he got surgery to become le Big Boss

There's a character creator you can use to make an avatar but as far as I know the canon avatar is that weird Medic thing that Ishmael has under the mask (with an bizarre hairline)

It's this simple
