Something that is unexpected but also has hints beforehand so players can figure it out if they're paying attention. It also needs to make sense and not be a twist for the sake of having a twist, the twist has to contribute to the plot. For example, using ZTD since it's in the OP, there are hints beforehand so you can figure out there's a 10th person in the game. What makes it a bad twist is that there's also other parts that don't make sense, like Eric and Mira never referring to Sean by his name and Sean apparently telling them his name off screen until after the reveal to hide the fact that he isn't Q. There's also Q's portrait in the status screen being a question mark when that's purely to hide it from the player. That's the game itself hiding information when everyone on Q team knows there's a guy in a wheelchair offscreen in every conversation.
Another Uchikoshi game with a bad twist is AI: The Somnium Files - Nirvana Initiative. In that game you play as both Ryuki and Mizuki to solve a mystery that spans over the course of six years. Ryuki's sections take place 6 years ago, while Mizuki's are in the present. Except when they aren't, every other day takes place in the other timeframe, so Ryuki has day one of his story in the past, the next day is in the present, and the next day is back in the past. Mizuki's is the opposite and the reason why she has the same model in the past is because you're playing as a clone of Mizuki who is also named Mizuki. Ryuki's days in the present work because he's a literal schizo and is suddenly thinking about events from 6 years ago. The only point to this twist is to be a twist for the player, they even have a side character explain the twist to you before the final chapters just to make sure you understand it.

The only bad part of the twist for me is the dialogue right before she leaves the classroom. I'm not sure if it's a translation thing but considering that it's Danganronpa I'd put money on it being a poor translation. There are enough hints in Kaede's behavior well beforehand that attentive players would pick up on the fact that she's planning a murder. The bigger twist of chapter 1 is that it plays the chapter 1 trope completely straight where someone close to the protagonist botches a murder and someone else ends up killing instead.

All of Star Ocean's reality is just an MMO. And it turns out the plot of SO3 is just the dev wants to cancel the MMO and shut down the server. You're all video game characters.They even went out of their way to ruin the other SO games by specifically saying those were just events in the MMO. Good meta twist. Four Danganronpa games spilling Pepto-Bismol everywhere got the games core audience to easily accept that pink blood is just a stylistic thing. But even if you do notice that the blood in chapter 1 was a more natural red, you quickly assume the weird rain probably is causing the pink somehow
