Would you want regions to include more fantasy-like elements or would you prefer they stick to the real world basis?

it would be a nicer way to give regions more unique identity again.
I hate that regions at this point have become "Oh look it's [Capital city} but in Pokemon!" and then every other city is an afterthought with 5 minutes of content. It's embarrassing that most players will remember non-consequential towns from the older titles and can't name more than 1 or 2 cities in the new ones. Focus on smaller areas again, and then fuse that with some fantasy elements.

The distortion world rly isnt part of Sinnoh but the other pictures r probably some of the most memorable locations in Sinnoh/Unova. I’d put Area Zero as a memorably fantasy area as well.

It's all just a veneer without any substance. There's no reason for those areas to exist besides series precedent, which, as much as I like it, is kinda what I get from Unovan route 4.

the Unova desert is by far the most out of place part of the region, they try to make it work with optional lore about how Relic Castle and the surrounding area was destroyed by the war with the dragons but they don't go into it deep enough for it to feel right IMO.

i loved the general sci-fi look gen 1 had to it, and like the general "weird environments" look gen 5 had to it when they did it right (such as the chargestones, etc.)
gen 5 especially because not everything needs to be explained just by shoehorning a pokemon in there. like if chargestone cave was made in later gens, there'd be like 1 chargestone but ooooh it's actually a pokemon charging the stone, no, just weird fucking electrical energy

I think it was more trying to look urban, and therefore more immersive, rather than it being "sci-fi."
