How do character texture files work? Do they draw this mess first and then put it on top of a model , or do they draw on the model directly and then make a file like this? GTA VC also had textures like this and you could open them in paint and draw directly over them and see it in game but it was difficult to make sense of it

Back in the day I think they'd create the model and export the coordinates layout to photoshop and draw over it, switching back and forth to make sure it looks as it should (hard mode)
Today they just paint right on the model (the gay ass mode)

Simply put, the coordinates of the "points" in a 3D model are laid out, as points in a 2D bitmap. In most cases that can't be done 1:1 for obvious reasons, and there's usually stretching and manually-defined "seams" involved. Even today, the process can only be partially automated, and it's kind of a pain.

Anyway, the artists then painted a bitmap with the wireframe as a general guide of where everything would end up. Still, painting on a flat representation of 3D geometry and have it look good was definitely a skill.

Eventually tools were developed to allow artists to "paint" interactive 3D models as if they were painting physical sculptures before 3DCG was even a thing, which as already pointed out is extremely gay and doesn't qualify as gainful employment by any stretch of the term.

It looks to be random data that somehow got into the texture and is being interpreted as bitmap data. The texture was probably ripped straight from memory instead of whatever file the texture assets are stored.

That's palettes. They're wrongly interpeted as 8bit texture data instead of 16bit color tables entries because the ripper can't tell

On top of what others have answered, it's worth mentioning some companies made programs for paining the model directly pretty early. In france Cryo made one as soon as they needed textured 3d, which was then propagated to most french devs on the sly because it was so handy
