Thy Flesh Consumed is kinda hard compared to the rest of the game. I've died more in E4 than the E1-3 combined.

Not him, but while I don't think Sandy Petersen was as bad as many like to make him out to be, his Doom 1&2 maps were kinda inconsistent in quality, especially in Doom 2 where he was kinda forced into making half the maps himself, and couldnt rely on Tom Hall leftovers anymore (besides map10).
I do love his Quake episode, though. Shows how much more consistent he can be when he doesn't have to make the majority of the game's levels.

The difficulty of E4 is a little overstated, though I can understand where people are coming from.
M1, M2, and M6 are difficult levels, with the transition to M2 from M1 being a rare case of continuous being harder than pistol starting, so that definitely gives off quite a first impression.
But the rest of the episode isn't particularly challenging, IMO. M4 and M5 are pretty easy going levels, as an example.

The strangeness and abstractness of his Quake maps feel like the culmination of the Lovecraftian theme of the game, like reality is breaking apart the castles and palaces into bizarre structures. There's a genuine terror in exploring those maps beyond the usual gory and gloomy imagery of the rest of Quake. Sandy knew his Lovecraft well so it makes sense

I think a big factor is that he simply had to make way more maps than everyone else. Of the original Doom and Doom 2 maps, Sandy made half of them, more than half if you exclude Thy Flesh Consumed.

He gets stretched thin, and it's most notable in Doom 2, where a few of his levels fall below his own average, though I still think he contributed a bunch of the best levels in both games, even if he also made some of the weakest ones too.

Many of his best maps in E2 are based on unfinished drafts left by Tom Hall, but his very first level was also E2M6, which is pretty good. No doubt it took that shape over some time, and not immediately, but John Romero was impressed by how quickly Sandy took to the software and the design philosophy, consider that during 1993, nobody had ever made a game like this before, the closest was Wolf3D and Sandy didn't work on that one, he just played it some. They were all shaping conventions as they went.
