Can't remember the name of a game you played, saw or heard about?
Anons will try and help you find that elusive title.
I'll start:
Horizontal shoot em up, but the gimmick was once you die you don't gameover but you instead switch into a more difficult "lane"/level of the game. IIRC this happens twice before you actually game over and each time you die the game becomes harder.

I used to have old CD-ROM homebrews of Pac-Man but they were very bizarre.

The levels were top-down had completely different stories. The graphics were very minimalistic and similar to the original arcade game but also used a lot of stock computer game graphics. It may have been made in
an old version of GameMaker? While there were mazes, where you eat pellets, I seem to remember it being more like Zelda but instead of using a sword, you use a gun. I vividly remember a driving section where you are in a truck shooting at oncoming enemies.

One of the games had a short intro where pacman's family is mourning over his grave, and then he rises from the grave with a title along the lines of 'Pac-Man is Back!'

There must have been at least 3 game discs, and a fourth disc with a christmas themed story. Does anyone know what I am talking about or is this more incoherant than usual?

All I can remember of this game is the starting menu being a greenish marble framed CRT screen with 5 gems of buttons underneath possibly all purple and maybe one red. At the time pre-rendered graphics were all the hype with DKC so the game was definitely VGA. The game was a mid-90s PC game, so it was more likely DOS era game. I may have inverted the colors in my head (so maybe instead of a greenish jade frame with purple gem buttons, it could be a purple frame with green buttons and one red). I don't think I liked it as there may be a possibility I had it and promptly returned the game.

My first guesses were Comanche: Maximum Overkill, but I liked that game, and 2nd was Spectre VR, but it doesn't look like it from any youtube video. There's no more hints I can give as this image is in my head, but I cannot for the life of me remember what it was.

This would've been around 20 years ago. And as for the console XBOX or PlayStation possibly? Not entirely certain on that though.

The game seemed nightmarish in tone, it looked like a bunch of men in white clothing on mats and there were machines over their heads and they were struggling. A similar vibe as I Have No Mouth and I Must Scream but I think it might’ve been Arabic themed or something

There's a PS1 3D beat'em up I just can't seem to find.

One of the playable characters is a gorilla and one of the bosses is an even bigger gorilla with a color swap. You were some sort of a task force and it supported up to 4 player multiplayer iirc.

It was one of those 16 bit games where you just downloaded a .exe from a website, I forget how I came across it
You were this little fella and you crash landed on an alien planet and you had to go collect parts for your ship
The world was very magical, mid 90s Nintendo, def influenced by yoshis island, magic quest, the lion king
You went underground a lot, there was a story, not a lot but you did sit and read sometimes when npcs would deliver exposition

Forgive this awful paint job, as it is my first time trying out GIMP. This is what I was describing, but with more of a prerendered look. The green is a bit more smooth marble, and those buttons were more gem-like. Maybe if I had some modern GPU I could AI specify better.

Decent guess as I remembered the ads for this game really catching me. It was how I learned of the stealth, but sadly that's not the game as the screen/menu (possibly main title menu) was pre-rendered instead of pixel art supplemented by polygons.

Pretty good, the UI has that pre-rendered look I was referring to, but my guesses were more implying that it was a boxed product that could be found in a store like Babbages, opposed to shareware or downloaded software. The genre may not have been vehicle combat nor flight sim specifically, there's a possibility it was an adventure game. I may have hit a wall with the game's complexity and not progressed much further than the screen that is haunting my memories.

Somehow the things on their heads look super familiar but it's probably just some vacuum tubes from a puzzle game or some shit in my brain. I would title your artwork "Giving Head on the Beach" though.
