Most people that actually enjoy DMC3 will end up playing through it over and over. The real "end game" of any DMC is trying to get high ranks on each mission on each difficulty. The game is pretty short for just a single playthrough even with cutscenes, after all. Though DMC3 is pretty brutal for getting ss-ranks on high difficulty. Not only for just orb counts, but on DMD just getting a single non perfect royal guard block is enough to ruin your damage s-rank. Still, it's fun working towards something hard.

You can't save/load, though, but if you're not interesting in playing the game twice it shouldn't be a problem.

This was only for the original release. US easy was JP normal, US normal was JP Hard, etc with the US edition having a unique hard mode being equivalent to very hard in the special edition.

Isn't difficulty just enemy stat scaling? Like who cares. You are probably already handicapping yourself by playing without healing items so what does it matter if the enemy has a bit more damage?
