What do i make of grades? Usually i think if i get 1.5 or more from common mobs i am doing the job right but i don't really know if i am correct.

Maybe i am dumb but i don't get how to do the second velvet's mystic arte. I pressed LT in 23 hits combo or even more but it still execute lethal pain, what am i doing wrong?I managed to do correctly a couple of times but i don't get it.
