Most quotes have a rough price / number of hours given by the artist who will then confirm at the end how long the job really took and exchange a product. I would assume the revisions fall into fixing the product for quality or additions that were forgotten. The pricing of that stuff does fall into a grey area. Your example of it takes longer than initially took to make the product means there was a major fuck up and generally customer is always right.
