what could cause this? where in the pipeline can this happen?

the trick is to limit your fps to juuuust below the amount of frames your gpu is capable of outputting, also if you're already doing this: some games don't play nice with rivatuner and you would be better off limiting it IN-game as opposed to doing it in rivatuner
