That scene takes place in the largest magic city in the region in the best restaurant in said city. Where did they ever show desserts like that in a "small town"?

Heck, Fern notes in an early chapter that while traveling she sometimes goes for months between being able to even eat anything sweet, let alone a fancy dessert. They are to some degree luxuries.

Frieren even remarks afterwards that buying that dessert cut into her savings quite a bit. That fancy dessert was extremely expensive rich people food.
