The introduction may have been a little bleak, but it's good to see where sensei came from. Not every story is going to have a happy ending, but there's hope to be found in many others. Hopefully you'll find them interesting.

That's not what the doctor is saying. He's saying that just as you can't heal a fever by saying you'll be there for the sick child, neither can you heal a drug addiction. Drug addictions are chronic illnesses. Willpower doesn't cure cancer; willpower also doesn't cure drug addictions. There's ample evidence that treating drug addiction like a willpower problem just makes people feel guilty and powerless and keeps them from seeking help. This includes words like "dirty" or "clean". Treating it like a cancer, where "using again" isn't a failure of willpower but something like a cancer coming back, has been shown to drastically improve outcomes, because when people use again, they don't blame themselves. They go, "My illness is back so I need to see a doctor," just as they would if their cancer came back, so they get help early.
The doctor said it in a dickish way, but anyone who seriously wants to help "fight a war on drugs" has to treat it like a medical condition and not a personal failing.

Re-read it. It's the first chapter, a prequel of sorts.
He tried to help the kid, but the kid had issues with the yakuza, so he started dealing, stole some cash. I guess Mizutani would've been fine with having the yakuza arrested and apologizing for the stolen cash, but he couldn't deal with a relapsing junkie. But worse, he betrayed Mizutani's trust when he admitted to selling drugs.
The chapter is about Mizutani learning an important lesson. Always be guarded against a junkie, just don't entirely close your heart to them.

It definitely can, there are people that succeed in quitting heroin or meth cold turkey, but those people are exceptional, and it isn't always because they're the strongest willed human alive, but because they're just genetically predisposed to getting off of it better than the rest of us. And sometimes it is because they just have a great will.
But there are plenty of others that just can't do so without relapsing over and over again.

Because they're not taught to do it. They're taught willpower, that weakness is worthlessness, that you should solve all your problems on your own, that being a "burden" is bad, etc. etc. Which causes all the problems in the first place.

A functioning society is antithetical to society as it is. Inequality is baked into the system because exploitation generates wealth. It's always better to ask for help, receive it, get better, and then return the favor when someone else needs help. But that would allow society to function, which isn't in the best interest of those who are in a position to give help.

It's very time inefficient but realistically you probably can't teach a damaged child the nuances.

Let's say I'm bullied at work. I know who I can speak with at management. If that can't be resolved I can speak with union or the ombudsman. If nothing can be done I'll probably arrange sick leave and use up my accrued ones and seek new employment. Or I can just have a holiday and chat with family, friends, a therapist. I might speak with my bank and rearrange my financial goals and probably use the time to get a health check up before finding work again.

How does one teach that to a child?
