The thing is, she serves as one of the villains of the last arc the anime adapted and is killed alongside her partner. The scene where she appears fat is from a hypothetical future the leader of the assassin kids views before passing away where all his subordinates live lives that aren't entirely perfect (here why she's fat) but are happier than the reality they faced, so she only appears fat in this brief scene (which is still better than nothing given she's still good-looking fat).Hope my post was useful.
