In inflation the subject is filled so much their body puffs out into a tight, taut sphere because that is the logically best form for containing the vast pressure inside them. (If you ignore the realistic limitations of the human body of course) WG completely misses this. A BBW is big sure, but that is just their normal, a fat person isn't in an abnormal, potentially perilous condition, they're just well...fat. It annoys me how WG tries to imitate inflation scenarios too, with characters skipping being stuffed full of food and instantly blowing up into lard whales from eating a bunch. Or WG bursting, which just never made sense to me due to the aforementioned lack of pressure in an obese form. Lastly, while I can separate fiction from reality in acknowledging that inflation in it's most fetishized form can never be a real thing, the same cannot be said for extreme WG. All you have to do is look up one of those docs about a 600 lb person to be reminded of how sad their lives are, and that personally just turns me off from fetishizing it.
