Any EM docs here?
What's it like in private practice? My only experience is at an academic hospital as a student, would love to know, how many procedures do you do in a normal day?
>>
how can things simultaneously be super competitive and there be massive shortages?
>>
>>16529842
EM docs have primary care practices? Huh?
>>
The most racist speciality is Opthalmology.
>>
>>16529877
Right. Obviously I meant a hospital that isn't an academic center.
>>
>>16529879
I mean. I think having a primary care clinic would be kind of relaxing compared to the wailing hoons of the emergency room. Plus no nursoids.
>>
>>16529888
Could always work at an urgent care, they're relatively cushy.
>low acuity bullshit with good RVU
>still occasionally get interesting patients
>send them to a hospital if they need further workup/management

Downside is they're staffed with an assload of mid-levels and you don't get the exciting resuscitation shit that gets people into EM.

Our EM clerkship director's husband does this, he works 6-8 shifts a month, 12-24 hours as a float doc for a few ERs and urgent care centers around town and makes like $260k/yr so he can take care of the kids while his wife at a teaching hospital makes bigger bucks working full time. If they want an extra vacation he just picks up a few extra shifts for a few months.
>>
>>16529899
Sounds cool besides the working wife part. Women shouldn't be working in my opinion. It's a leading cause of infertility. Same with school.

Limited number of medical school admissions.
>why are they limited then
Because maintaining an artificially low supply maintains high demand, which in turn maintains high wages. Not to mention it protects old fucks from getting replaced. It's all about protecting their interests.
>>
>>16530173
The real rate limiting factor is Medicare allocations for residency pay. Congress controls how many residency spots there are. If we made 3 times as many med students without increasing residency spots, it would be a huge mistake.
>>
>>16530205
why don't companies just create more residency spots? does it have to go through the government?
it's not like they aren't profiting from residents working 30 hour shifts while getting paid piss

as it stands it seems there's a lot of potential talent going to waste
>>
>>16529814

>>
>>16530205
Residency spots is a factor too but i feel like med school is a bigger deal. Failure to match sucks but you are still a doctor after graduating and will match somewhere eventually. People that don't make it into med school to begin with usually fuck off to do something else and never return.

>>
>>16530306

>>16530173
I'd rather an influx of GPs than mid levels. The mid level cancer we have resulted from no route-specific schools. Like dentistry, all specialities should have their own schools. For example: surgery that branches into specs; IM that branches into specs; etc. Likewise, there should be GP schools. Maybe IM wasn't a good example because there is primary care IM
>>16530205
This. However, research hell would be an outcome if there were no residencies. I think we need more clinical PhDs to allow the antisocial autists minimal patient contact while also directoy benefiting patients (e.g., pathology).
>>16530209
Because everything is fake and gay. We need more physicians and surgeons. We need zero washed up cheerleader nurses, and we need to go back to allowing only nuns to be nurses. Hospitals were better back then.
>>16530282
Looks fucking cool.
>>16530353
Those positions never should have existed. We need more physicians, not more NPs who do cardiac surgery (MedScape Citation -- I'm drunk fuck you).
>>16530389
I'd be bitter if I was a mid level who had MD intellect but was filtered by politics or the retarded nu-MCAT. Instead of the MCAT, there should be an IQ test, and psychologist administered personality inventory. GPA is enough. The damn GRE was abolished, and in some places the LSAT was. The MCAT doesn't predict anything (under 500) other than performance on same-company exams. It's a money-grab.
>>
>>16530429

A product of your generation.
>>
>>16530601
You're right, we should actually make them higher. Make it so only the top 10% of current med school applicants make it. Who cares about shortages, anyways? I'm sure we can make it we 10 times less doctors.
>>
>>16530615
i prefer my doctors to understand mathematics, chemistry, biology, etc. before they ever set foot in med school, thanks. Also, you might be happy with the bottom quartile getting degrees and manging your kids' health, but I'm not. Lowering the standards is a stupid idea. You're stupid. 'But no doctors tho' isn't a good enough reason to debase the profession.

ironically, standards are higher than ever due to competition

if anything, we should make older doctors take exams to maintain their licenses and keep them up to date
>>
>>16530601
EU docs don't need a useleww undergrad. It's high school -> med school. Are the Euros shitty doctors? Not even close.
>>16530835
AAMC Jew detected. Medicine used to be cool, and hardly anyone wanted to do it. It's artificial desire that people develop from le télèvisiön. People want to be called doctor, be distinguished, have praise, and have money. Most people seeking medicine don't give a damn about medicine. They should have gone to be finance bros instead.
>>16530636
So, the GPA inflation for undergrad isn't producing lower quality students? Do you think everyone retains math, bio, chem, etc, knowledge after they finish the class? Newsflash, bucko! Nobody who gets into medicine even remotely cares about basic sciences. If they did, they'd pursue basic sciences and not medicine. This retard thinks only Mega Mind™ applicants should get in. Cool, and in your world full of autists and aspies, nothing gets done, and nobody wants to let the physician touch them.
>>
Med's razor: mid levels are encroaching because of low physician numbers; low physician numbers because "muh quality" must be unrealistically high.
>>
>>16530940

>EU docs don't need a useleww undergrad. It's high school -> med school.
I'm a med student in EU, keep in mind our med school is typically 6 years, instead of 4 undergead 4 mech school like in US. Still 2 years ahead though.

Also my high school exams to get into a med school were somewhat tough
>3 exams, biology, chemistry, subject of choice (i took math because most predictable easiest to 100% imo)
>basically needed a total of at least 240% out of possible 300%, so an average of 80% from all exams, with a hard requirement to have above 80% from biology
Dunno how hard undergrads in US are but in my med school passing grades are set at 60% usually, so over 80% to even think about getting in was somewhat stressful. I knew a few people who needed another year of studying on their own to get in because lmao public education
>>
>>16531064
To get into a US MD school, it is advised to have a GPA of 3.7 or higher, and a science GPA is even more important. You are also required to have many prerequisite courses in the basic sciences and some advanced science. On top of this, you take a difficult exam (one on which good performance has been directly shown to increase pass rates of board exams later on). Then there's a difficult and expensive application process, interviews, etcetera. It is much, much harder to get into american MD schools than EU ones.

>t. knows many international medical graduates

I'm not saying that US MD programs produce objectively better doctors, by the way, just outlining the process. Also the above anon is right. Prereqs don't matter and the MCAT is nothing like medicine, it's just a benchmark to see if students are able to grind a bunch of dumb shit they don't care about, since they'll have to do it to pass STEP and Shelf exams.

if all of this is "that easy" and public science source data it'd have been tried hardcore as an adjunct therapy technique for people who have recently lost limbs.

The mere idea that it's possible to "recode epigenetics" back to a "stem cell" or pseudo state and this will magically fix DNA but above that macrostructures is outlandish. Not that the chemicals create activation situations for other chemical responses, but that it would turn back time.

Tell you what I'll gather up a curated blend of supplements and let you know if my arthritic finger gets better in 12 months or so and if my hair loss stops.

I graduated with a 3.8 GPA (b/w A- and A average) while studying in a woke university English department and Neuro department. To this day idk how I survived the English department. In high school I didn't study and got 80s very easily. This just proves my point. Personally, I'd prefer a 6 year MD format for NA. That, or a 4-5 year MD if you already have an undergrad. The MCAT is a shit test and it should be trashed like the GRE.

I know an old fart EU (Irish) doc and he said when he was growing up, MDs weren't praised like they are today. It's because MD != PhD, among other reasons.

With Apigenic Health you would be activating Oct4, Sox2, and the Myc-c genes. Its in the theoretical wheelhouse, but I agree we need a heck of a lot more data than we have at present. Are you male (penis) or female (vagina)? I'm assuming male because getting to 5,8% as a female would be very hard, but gotta ask regardless.

If male, this is unhealthy and you can expect hormonal imbalances/weakened immune system (lower test, less muscle, weaker bones, being tired all the time, getting sick easily, struggling to think, etc.) after a few weeks or a few months, depends on a lot of factors. You shouldn't be going below 8-12%.

If female, this is VERY unhealthy and you should expect the above plus more plus much worse plus much faster. Your period will stop most likely, but you'd probably experience this way before getting to 5,8%. Shouldn't be going below 20%.

I'm assuming you are bodybuilding and went to a hard cut to get this low, you shouldn't be cutting this hard unless it's for a comp. I mean you shouldn't period but if it's for a comp then I get it. Mirin tho
>>
>>16534007
How about you go to medical school and learn how to do a focused physical exam pertinent to the patient based on your differential.

You do have a differential, don't you?

To answer your question, ask your supervising physician what they think is appropriate. If you're unsure, do a reasonably thorough physical addressing all systems that you are concerned about based on demographics, history, and chief complaint.

Male (penis). I'm no bodybuilder, I just wanted a leaner face
I literally did a slow natty cut with a small caloric deficit over months plus 1g/lb of protein and exercise, losing something like 7-8kg. I never thought it could get this low. Apparently something like 52% of my mass is muscle, blood tests are ok, everything is ok minus my body fat which seems to be shockingly low
How accurate are those tests? They made me stand barefoot on a machine and grab some bar in front of me while fasted
>>
>>16534131
>How accurate are those tests? They made me stand barefoot on a machine and grab some bar in front of me while fasted
Range from decently accurate to very inaccurate, depending on how shitty and old the scale is. DEXA and hydrostatic are the truly accurate tests, get one of those if you want to be sure. Regardless, even an inaccurate reading of 5,8% implies that it's at least around an unhealthy range.
> I'm no bodybuilder, I just wanted a leaner face
>I literally did a slow natty cut with a small caloric deficit over months plus 1g/lb of protein and exercise, losing something like 7-8kg
Well people have different definitions of what a "small caloric deficit" is, just like fatfucks think gorging on 6k kcal must be normal. That said it's hard to get to 5,8% on accident, you must have been pretty lean and not eat a lot to begin with, or maybe you're doing a lot of cardio (running, swimming?). I'm assuming you're counting calories, how many are you eating per day?
>blood tests are ok
As I said, it can take up to a few months, it varies, depends on genetics, acitivity, etc. It shouldn't be a big deal to be this low for a few weeks, even a few months you will be fine long term, it can only become a real problem if you are like this literally all the time, comorbidities, etc.

No you don't, nobody does

It's insane that we're still expected to obey the argument of "doctors know what they're doing" when it wasn't that long ago that they were lobotomising people

"Oh, now we know better"

Absolute bullshit. When clozapine was invented (a drug that is still used), it killed people. It took them a while to figure out why it was killing people.

The attitude of doctors is "let's just try this treatment, and if it harms the patient, I couldn't give a shit, because it's not harming me".

My question is this, I've had a pussy toe like 50 times because of this ingrown nail but people act as if this is terrible and incredibly dangerous
Why can I have an infection there and not have any problems with it?

Is it weird that my dad insists on moving with me to medical school so he can "support me"? I've never truly lived away from home, my undergraduate was mostly spent at home, I spent a few semesters on campus but came home every single weekend. I still don't know how to do laundry or anything. At this rate I will still be living with my parents when I'm a resident physician

Is biliverdin responsible for the cyan coloring of this lizard's mucosa or no? They're supposed to be "green" on the inside due to massive bilviderin buildup. The blood samples look pronouncedly green in the plasma, but the heavier elements look black with a tinge of blue, rather than green or red.

There isn't many of these lizards and even less photos AND they're often mislabeled (this one is supposedly prasinohaema prehensicauda), so it's impossible IMO to eliminate on the basis of comparing visually on the internet.

The first guy I saw at that same clinic (actual psychiatrist) put me on the Wellbutrin + Celexa stack, and shortly after I started the Celexa (a month or so after Wellbutrin since it didn’t do shit) my entire worldview changed. I woke up one morning and finally felt “normal” again. It was insane.

An unresponsive patient improved after the administration of opioid-antagonists. That happened a few times. The kicker: he didn't receive any opioids. How can that happen? This was in a hospital setting, so he definitely didn't get any opioids. How come he improves drastically after receiving them?

I tried to look for similar cases online, but I don't know what to search for. Entering those keywords just gets me general opioid receptor and antagonist articles and results.

I assume it was part of the guidelines for sudden unresponsiveness without any apparent reason? I wouldn't know, I'm just a nurse. It happened in the ICU, where he was transferred, I think 3 times, for the same problems while being on our ward. Unfortunately I was never involved, but only heard a colleague mention how the ICU people are angry that it keeps happening and that they won't take him in another time.
I think they've settled on treatment limitations with the family at one point and that's why there weren't any detailed inquiries into it.

I just found it interesting and was wondering if there could be another reason.

>slow descent into madness
It can be if you’re not competent enough to develop your craft on your own without them. Once you know what it feels like to be confidently productive, the feeling of sobriety seems insanely hollow compared to it. Being on them and then having them taking away prematurely is an actual descent into madness.
>>
>>16535385
The way to go with all performance enhancing drugs is to reserve their use only for critical events where performance matters the most, that way you get the biggest benefit with basically no drawbacks. And yes, that includes caffeine.
>>
>>16535475
I have a hard time agreeing with that entirely as it depends on the effectiveness of your dosage. If you’re getting the results you need from the doses they’re prescribing then it’s unlikely that you’ll binge on them for any reason (which will indeed build a massive tolerance). These stimulants require building a lifestyle around them to ensure maximum effectiveness.
>>
>>16535506
These stimulants are meant to be taken regularly. Having 0 tolerance whatsoever can make the effects unpredictable and will most definitely result in a miserable comedown.

>>16535489

Ever heard of allergies? CMPA is especially common in young children and the kid is going to be DEVASTATED when you offer them chocolate and their parent has to say they can't have it. Lollipops are fine, food coloring allergies are rare, but i wouldn't bother.

Bring stickers. Can't go wrong with stickers. Chances are their parent is gonna get them a treat after anyways, but only you can give them a sticker.

Do ears last, kids usually hate it.

If the kid is scared of anything (like the otoscope, again they usually hate it), offer to do it on their parent first, usually calms them down.

I'd recommend against it. Complications are actually somewhat common at 1-2%. This may not sound like a lot, but if i offered you a bowl of 100 candies but told you one of them will make you go blind, you probably wouldn't want to eat from it. And when they happen, you're absolutely fucked and i do mean FUCKED.

LASIK creates a flap in your eye that never quite heals completly, so you will forever be at a risk of dislocation from any trauma or even just strenuous activities (parachuting, sports, etc.) and if it doesn't happen, you can expect to be legally blind in that eye for the rest of your life.

There's also a decent risk of starburst, glares, halos, problems with night vision, etc.

If you REALLY want eye surgery, I'd consider PRK first. It's an older procedure that doesn't involve creating a flap. It heals for much longer, but i believe it's well worth the lesser risk of complications. Military pilots get it when they need eye surgery, for example.

That said, if you can wear glasses, just wear your fucking glasses. If you really don't like glasses and don't have issues with eye dryness, wear contacts. Eye surgery should be the absolute last ditch effort when nothing else works.

https://www.drugs.com/drug-interactions/celexa-with-wellbutrin-679-335-440-203.html

Drug Interactions between Celexa and Wellbutrin
This report displays the potential drug interactions for the following 2 drugs:

Celexa (citalopram)
Wellbutrin (bupropion)Drug and food interactions
Moderate
buPROPion food
Applies to: Wellbutrin (bupropion)

Using buPROPion with alcohol may increase the risk of uncommon side effects such as seizures, hallucinations, delusions, paranoia, mood and behavioral changes, depression, suicidal thoughts, anxiety, and panic attacks. On the other hand, sudden withdrawal from alcohol following regular or chronic use can also increase your risk of seizures during treatment with buPROPion. If you are prone to frequent or excessive alcohol use, talk to your doctor before starting buPROPion. In general, you should avoid or limit the use of alcohol while being treated with buPROPion.

Interactions between your drugs
Major
buPROPion citalopram
Applies to: Wellbutrin (bupropion) and Celexa (citalopram)

Talk to your doctor before using buPROPion together with citalopram. Combining these medications may increase the risk of seizures, which may occur rarely with either medication. In addition, buPROPion can increase the blood levels of citalopram, which may increase other side effects. You may be more likely to experience seizures with these medications if you are elderly, undergoing alcohol or drug withdrawal, have a history of seizures, or have a condition affecting the central nervous system such as a brain tumor or head trauma.

>This was across 1-2 weeks, with different nurses on duty. The likelihood of different nurses accidentally giving him opioids is very slim.

You're thinking like a doctor (rule following dumbass) not a human or a parent.

Women are shitbags. The slightest discomfort causes them to become violently hysteric and neurotic. For popular examples, consult "women who cannot get dates" YouTube vids.

So what does a negligent mommy type do when baby cries and she doesn't want to interrupt her reading? She puts some booze in the baby bottle and gets her toddler drunk. Then she can whoremaxx on romance novels while her child gets brain damage.

Nurses are the laziest whoremaxxed cuntoids out there. Patient cries, "my rib is poking into my lung", she slips him some opiates from her stolen stash from the hospital supply (she swaps out other patients painkillers for aspirin or Tylenol and pockets bottle after bottle of "happy pills") then if the patient dies, it happens off her shift, she never gave a fuck. MOST NURSES ARE LKE THIS. Other than that it's 8 hours of robbing patients, beating patients, deliberately poisoning pretty women and unattractive men, playing god like the shittiest mommy. Stupid literal druggie whores in med school, stupid literal druggie lazy whores in the hospital in nurse costumes.

Anyone deal with rumination syndrome? I’ve been seeing BH and a GI over a year and no improvements I’ve been regurgitating 20-30 times twice a week and about 10 times normally a day to the point where I’ve been losing weight not being to keep anything down. I’ve been using diaphragmatic breathing and 40 mg baclofen throughout day without much improvement.

I know I shouldn't smoke (especially as a medical student) but it makes me feel alive.

Oral fixation.
>>
>>16539535
Staying on this earth longer than we have to is gay. Smoking is fine. I just got 200g of pipe tobacco that showed up today.
>>
haven't done much research on it yet, how's the nicotine pouch situation looking as far as risk factors? anyone encounter anything definitive yet?
>>
>>16539698
Pretty sure there's little to no research on long term health effects. No smoke, no tobacco and no inhalation, so you're skipping most if not all cancer risk already. I only remember pure nicotine being associated with cardiovascular diseases.

Keep in mind absence of evidence is not evidence of absence. It's probably the best way to quit smoking if you can't do it cold turkey, but i wouldn't call it a safe habit.
>>
>>16539705
yeah, and there's no way the constant abrasion of mucosa and vasoconstriction won't be damaging long term, i'd imagine that in itself predisposes to gum disease if not carcinoma.
>>
>>16539698
completely anecdotal but I knew a guy so insanely hooked on zyns it pretty much seared through his gums and he needed surgery. He must have been sucking on 2-3 containers a day
>>
>>16539778
oof
i can't imagine that man.
I use velos because they're cheaper, and if I use more than like 3 in a day my gums hurt the following morning. I'm working on quitting (still costs too much), switched to a lower nicotine dose and limit to 2 a day and it's been working. I'll have this beat by the end of January.

Thoughts on 1 year master's programs with linkages to medical school?
My GPA is incredibly mediocre, about a 3.0 (I failed English and some other easy classes because I was going through depression and some very difficult circumstances), cGPA roughly a 3.3-3.5.

Look up bladder training. You can try doing kegel exercises and also try to slowly increase the time between peeing. I also remember reading something about voluntarily starting and stopping mid stream while peeing to improve your muscle control, but I'm not sure how good that is for you long term.

If you can't get into medical school, that's on you. Getting into medical school isn't as hard as you think. At least not for idiots like you.

> no way the constant abrasion of mucosa and vasoconstriction won't be damaging long term
As far as I'm aware you don't chew on nicotine pouches or anything like that, they only lay against your gums, so abrasion should be minimal, especially if you regularly change the site of application. Chewing tobacco is known to cause gum disease and mouth cancer, but tobacco itself contains a lot of carcinogens and the act of chewing does cause it to scrape against gums. It also sticks to gums and promote plaque/tartar buildup. Still, nicotine itself can irritate gums, and there is some abrasion, so you may be right.

every time I post here I am harassed.
I haven't applied to medical school yet. Application cycle opens up this coming spring and Ill have a year off so why not do a 1 years masters with linkage in case I get rejected (if i get rejected it will be because of my gpa)

there are many linkage programs that say "guaranteed interview" which is a ruse to humor you and justify them robbing your money. They essentially put you at the bottom of the list from the regular applicant pool since your interview is out of obligation.

Focus on getting a decent MCAT above all else
>>
>>16540117
My MCAT isn't a problem. In fact, my entire application is good except for my mediocre GPA. You think these programs just rob you? I know a lot of people who did the program and got right into the medical school. If I don't do the program then my life is just going to be taking a year off doing nothing or galivanting around in Europe partying and having prostitutes and getting drunk. Why not do something constructive during that time like a 1 years masters?

Give up? Apply to medical school and see if I can get in (which I probably won't right now since my GPA is too low)? Or don't you think it's wise to FIX MY GPA and instead of spending all the money on more courses at my uni just go do a masters with linkage in a new location in a nice place where I can lock in
or just give up and kms
like what the fuck guys

>everyone around my age has their shit together
How would you know this though? Do you go around asking people if they have been in the same profession since they finished high school, if they ever dropped out of uni/college, etc.? Most people seem like they "have their shit together" on first glance, it's not hard to fake as long as you're not literally homeless. Hell you can even fake having your shit together while homeless if you're not a total drunk, unwashed hobo. Don't get me started on social media, either, people only share what they want to share, they will either strive to be seen as successful by only sharing the good side of their life, or share their bad moments to harvest sympathy and attention.

Anyways if you want a reality check, i'm 29 and started med school a year ago. I dropped out of uni twice before. I spend a few years working until i realized med is what i wanna do. I cope by telling myself I' still gonna be practicing longer than the average doctor because the average doctor is gonna become a sedenary fatty with a smoking/drinking habit at some point, i plan to outlive them by at least a decade.

I don't know Anon, this variant seems pretty concerning...
We've had some pretty horrifying omens like the hundreds of dead Sea lions washed ashore infected with the virus. I know that the Covid pandemic was a lot of overhype and bullshit from all angles but it's not any reason to ignore this. It's not as if we haven't seen what could happen if a disease like this runs rampant.
Especially one that could jump from animal to animal and then to human to human.

I mean fuck, i'm not even all that worried about lockdowns and all of that shit I am worried about the animal populations that could be absolutely devestated. BirdFlu has already shown that it can spead to and kill cats and dogs. They don't deserve any of this shit.

Go to the bathroom even if you don't feel the urge to urinate. I had pantswetting as a kid and the culprit was chronic urinary retention (because I didn't want to get up from the computer) - the high pressure of keeping my external sphincter closed basically caused my bladder wall to concentrically deform (get thicker) and my detrusor muscles to spasm

Far stranger things abound in medicine.
>don't see how literally half of that hambeast's finger could grow over a ring
The finger was grown when she was a fetus. Sorry to be pedantic, but it is part of the explanation. If something receives blood, then it can grow and become filled with fat, assuming the diet allows for it. Here evidently the arteries that feed the finger are not being tightened by the ring (that's how she originally put it in the first place, otherwise she would immediately feel discomfort), thus it can grow.

>>16540705
"White" america can't hack it through life without those. Pill mills are the thin fine line that makes us rulers of the planet.

health information management, clinical care seemed cucked to me (originally went physical therapy route) and I'd rather stay seated so I can die from pulmonary embolism or diabetes sooner.
