The Starks and Lannisters need no dragons to keep their own vassals in check. You could also ask why the Reynes didn't just overthrow the Lannisters. If it's a running system, then there's lots of people who benefit from maintaining it on all levels. I mean irl kingdoms also work(ed) without dragons. I think what's more unrealistic than 150 years of dragonless Targaryen rule is 10.000 years of Stark rule or whatever.
