Depends on your income, insurance status, and what sort of hospital it is (and also what you had done). I'll give an example:

It is true that in America many hospitals operate as non-profits, which means that they can write off your bill as a loss. The term they use for this is called "compassion". Pretty good, right? But there's some caveats.

This typically only applies if you are low income/no income, and no insurance. It also typically applies depending on what time of the month you grab them. They have redlines or "pools" they can service costs from for that month or week. If you've ever worked customer service, you might be familiar with a much simpler version of this where you are allowed to write off a specific chunk of money per month, but nothing above that. Same thing here.

Finally, a lot of hospitals have private practices inside of them -- and these dudes will not go away, and they don't play by the same rules. To give a personal anecdote:

I had to go to the hospital for sudden afib, which was unusual for my age and lack of medical history. I got there, and the after sitting in a bed out in the hall for about 8 hours they let me go with lots of shrugged shoulders and no answers. During that time they sent down a woman from their financial office to speak to me about the bill FROM THE HOSPITAL. It totaled up to something like 4 grand, but I was broke and young, and had no insurance. So they gave compassion and wrote off the bill. Logically, you'd think you're in the clear because of that. NOPE.

They did a chest X-Ray, the X-Ray department was a private practice in the hospital, so those guys sent me a bill for $2500 which they wouldn't budge on and would send to collections. That took me a year and a half to pay off. Then there was another "got ya": the EMS. Those guys are separate companies. It's why Americans take Uber to hospitals (I've done this), it's always cheaper and usually just as fast. I had to pay them $750 to go 5 miles.

A lot of practices already don't send the bills to collections unless they grind you as hard as they can and get nothing. The reason being: collections buys the debt for pennies on the dollar, the doctor or hospital or whatever has already determined you're a dry well so fuck you, hit your credit score. This is bad because their billing departments want to try and extract the maximum amount of money possible, so what they do now is send your bills to "collections agencies" who act on behalf of the hospital/doctor/whatever and tell you up front that they haven't hit your credit, and they won't hit your credit so long as you pay off the bill. Why? Because people are a lot more receptive to the idea of putting money into the system if they know you (the hospital/whatever) haven't just fucked them sideways on their credit. I have a medical debt I am fighting right now for bloodwork that came out to $3500 on a routine physical that wasn't covered. The collections agency told me flat out what their procedures are and that I have literally 24 months to make arrangements before the diagnostics firm gives up and sells the debt off and hits my actual credit score. Right now I am just trying to sort out what the fuck my doctor actually ordered and why. It was supposed to just be a regular metabolic panel, but they did an insane amount of screenings my insurance won't cover.

Depends on the circumstances and a couple other factors: I had some major surgeries a few years back and the hospital was actually really good about negotiating with my work insurance to get the bill down, and making a reasonable payment plan (which I only ended up having to cover about half of before they just wrote off the rest). What really sucks are private companies that hospitals contract with to help with the workload on labs and shit - I was getting letters from companies five or six years after the fact to pay off shit that had been missed by insurance. In those cases, it's basically a matter of how much you own and whether it's worth it to them to try and compel payment. A couple grand? Oh yeah, they'll take you to court over it. A couple hundred? Yeah, no, just ignore the texts and collection threats, it's all pretty hollow. Nobody' taking you to small claims court over a $100 bullshit lab fee.

It's kind of a fucked up system, but historically it's had some positives to it over a universal system. Most places are usually pretty good about getting you in for voluntary procedures or to see specialists pretty fast compared to Europe or Asia, and outside of major cities the wait times aren't usually that bad unless you're down in the ER.
