I got to thinking about something similar the other day, and came to the conclusion that:

When I was a very young child, in the 70's, we played with legos. We had big box or container, something that had passed down through 1/2 a dozen children already, and we just made stuff.

We weren't told what to make, or how to make it. There was trial and error, and mostly the imagination. You can (and did) make everything; and when you were done, you'd throw in back in the box, ready to be torn about the next time when you wanted to build something else.

Now, not only do they have to tell you what to build, they have to provide you with idiot-proof instructions on how to put the simple parts together. Meanwhile, the child who built random crazy stuff for no other reason than creativity, is replaced with a child (or adult), who want the thrill of being creative without the burden of being creative.

I could go on, but another example would be "The Beam", where people can pretend to be as brave as 1930's workers eating their lunch suspended in the air; all in the comfort of an entirely safe (but fake) photo-op tourist trap

Interesting to see how much ENT dominates the list, overtaking VOY & TOS and nearly tying with DS9. I imagine this can be attributed to ENT airing at the height of IMDb's internet relevance, before all the big social media platforms.

One of the main things I'm curious about is seeing what fans consider to be the best "regular" episode. As you can see the top of the list is dominated by what you might consider episodes with "inflated" scores due to mets reasons (i.e. all the TOS episodes that have entered the public consciousness and have thus become harder to gauge an objective scoring of, episodes that are important to the series due to lore reasons like Q Who, episodes that have a narrative twist/gimmick that make them stick out more like The Visitor, etc.).
By these standards I reckon Cause and Effect is the highest "regular" episode on this list. I think you can definitely argue that it's one of the most "Star Trek" episodes of all time.

Another thing I'm curious about is seeing if general consensus on an episode changed over time. I feel like Darmok used to get clowned on when I first got into TNG, but nowadays I'm seeing it praised more and more as embodying the ethos of Star Trek.

The Tom body horror stuff is the best part. Can cut out the Warp 10 stuff as that part of Trek lore feels too important to fuck with in a one-off episode and definitely cut out the baffling ending.

My idea for a fix would be to sort of combine the concept with Favorite Son and the Kobali episode:
Tom fucks an alien, turns out this species is hermaphroditic and reproduces by laying eggs in the loser (similar to snail love darts), this also mutates the host not unlike parasitic barnacles, Tom's body starts mutating just like in Threshold, the Doctor can fix all this but doing so would kill the babies inside him, Tom refuses, ethical dilemma about whether that is really Tom, eleventh hour risky technobabble allows them to restore Tom without killing the babies. Maybe some discussion about whether they should share this breakthrough with the aliens in order to end the suffering caused by their reproduction or if completely fucking with a species's reproduction methods has too many ramifications.
