I'd imagine there's little build up since it's basically just an expanded version of chapter 3 from the original, so you're basically missing the entire beginning of the game. Anyway, it depends on what you want. The original is pretty much a big adventure story and while it has a good amount of porn, it's not the main part at all. The gameplay is pretty simple, so if you're not in it for the story then I'd say it's not worth it
