Well, maybe he had a secret room with a reasoning why the quest was there in the first place. Play a detective role, figure out everything yourself. There's million ways to make it and only lack of imagination and time constraints are limiting devs to implement everything. Of course we can also go without killing NPCs because you're aligned with good and game simply won't allow you to attack anyone, but the choice can also be done by accepting quests, declining quests, failing quests or finishing quests in another way. I played Deus Ex HR and there was a questline where you have to stop a corrupt cop. This corrupt cop gives you a quest to start a turf war or something. I was actually bummed that you couldn't start that war. It's not what Jensen would do but the option should be there in your RPG game. Anyway there are ways to continue quests when main people are killed, but I'd like to think that branching paths should swing you from chosen one to the chosen evil one and all shades in between.

It's important to define failure and why failures tend to believe in things like luck.Failure doesn't mean "I was making a game but then I quit". It means "I made 10 games and they all failed".
