It forces Fatima to eat the corrupted garbage to keep it alive and uses her as a shield against light and life. It's also why Elgin forced Fatima underground so Smiley can be properly born again. It does seem like From is a battle between the realms of life and death, literally.

The reason why it's Smiley again is connected to the old rituals. Maybe every baby is destined to die so that a monster can be born again if a monster dies? Or something. It's connected to the timeloop and how the monsters are seemingly invincible and just keep coming back.
